This has finally been fixed. FYI there's always been an index.php page in the site root. It just wasn't handling a certain scenario properly.

I haven't tested this since I don't have that style, but here you go. Unzip and put contents in the extension's styles directory.

The folder that your style is in is Ravaio?

It's likely not inheriting from prosilver so pretty much expected that it looks like that. What style is it? Just a matter of creating the template files for it.
